How does someone get medical care without insurance or a way to get it?

My friend suffers from a debilitating back injury. Recently she lost her insurance and was taken out of work by her doctor. However the process of getting disability is proving to be extremely difficult. When she can get in touch with them, they seem to have always lost her paperwork and are going to call her back. They never do. Now she has no medication to ease the pain, and is in need of a major surgery to prevent her from becoming paralyzed. When we tried to sign her up for the Affordable Care Insurance, they said she did not qualify for any of the discounts because she has no income and no family of her own, so they were asking her to play 400-500 dollars a month for insurance, and she can't even work to obtain money to pay for it. Every day I watch her go more and more in pain, with no way to get any relief. I have done all I can to help her, but there really isn't anything else I can do. How is someone in her situation supposed to obtain medical care when they can't afford it? Are they just expected to live in extreme pain for the rest of their lives? As an added note, we live in a state that did not have the Medicaid expansion either. We really have tried everything.

before stolen: smooth drive, nothing wrong with it.. after stolen recovered: insurance says it will cost 2300 to repair it hence offered me 'total loss' and got me a 'salvage offer' of 4k. Why the health insurance are different than car insurance?

I mean insurance is protection and give you money when you got into situation that cause loss or expenses. But why car insurance are less picky and more willing to give money to pay out than health insurance? Why more rules and more restriction and more reject the coverage with health than car? I mean I understand insurance is business and for profit so they has to refuse the claims if loop hole. But it shouldn't be any different on health or car coverage.

I've had the following insurance and was not pleased: -Insure One (real insurance: Access American Casualty)=Cheap but HORRIBLE CLAIM PROCESS -Farmers Insurance Group: Pricey and more hidden fees hence had to switch because I felt like I was paying too much and getting scammed (I tried resolving the issue but rep said there is nothing we can do ). Why do insurance rates vary so much between states?

Sometimes states that are right next to each other have totally different insurance rates from the same company for the same car and driver. It's not like driving magically becomes more dangerous when you cross the state line, so why does this happen?

I was in my first EVER car accident and it was my fault.
